    Appointment Information / Scheduling

    Please complete the form below in order to schedule an appointment. In the message section, please indicate if you are planning to use health insurance, and, if so, the name of your insurance company.

    In general, I schedule patient appointments during the following times:

    8:00am – 6:00pm Monday through Friday

    8:00am – 12:00 noon on Saturday.

    (PLEASE NOTE: The early morning, early evening, and Saturday appointments are typically in high demand and tend to fill up quickly. If your preference is to schedule an appointment as soon as possible, having some availability for sessions during the day is encouraged).
